LPN Full Time-Days (8am start) No Weekends or Holidays Calumet Medical Center, Chilton WI The LPN is responsible for performing delegated aspects of age-appropriate patient care, including data collection, advanced technical skills and monitoring, under the direction of the provider. The Licensed Practical Nurse (LPN) contributes to the plan of care, communicates professionally, reinforces teaching and serves as a member of the interdisciplinary team using a holistic, caring approach to personalized patient care. Duties: Age-appropriate direct patient care under the direction of the provider according to established policies and procedures, in order to carry out the individualized, interdisciplinary plan of care for each patient. Documents accordingly. Assists the provider and reinforces teaching of patients/families, staff and students. Prepares patients and record for visit. Verifies route slip for accuracy. Administers medications and immunizations. Instructs patient/family and reinforces provider education Receives telephone calls, responds and documents appropriately Forwards diagnostic data to providers and responds as directed Works in conjunction with supervisor regarding patint and provider relation initiatives and works to maintain hospital/clinic relations. May work in designated areas such as specialty services, family practice and orthopedics as business need dictates. Qualifications: Job Requirements Minimum Education Required : Graduate of an accredited school with Wisconsin State LPN license. BCLS certification. Minimum Related Years of Experience ( per minimum education ) Required: Outpatient clinic experience preferred. Demonstrated skills in establishing and maintaining effective working relationships with patients, physicians, staff and customers. Able to react calmly and effectively in emergency situations. Able to read, understand and respond to detailed oral and written instructions. Ability to communicate clearly. Job: Licensed Practical Nurses
