GENERAL SUMMARY: Under direct/indirect supervision, Rehabilitation Assistants provide necessary and appropriate personal care and community living supports to persons with developmental disabilities, persons with severe mental illness, and/or individuals with co-occurring diagnosis. The Rehabilitation Assistant provides suitable services with respect and dignity to all persons-served. The Rehabilitation Assistant has a high degree of personal integrity, generates positive energy, and makes a difference every day. The following statements are intended to describe the general nature and level of work being performed. They are not intended to be construed as an exhaustive list of all responsibilities, duties and skills required of Rehabilitation Assistant. Some duties described herein may not be assigned to every Rehabilitation Assistant.Weekly: Sun, Wed and Sat - 3:00pm to 11:00amTYPICAL DUTIES AND RESPONSIBILITIES 1. Provide supreme customer service, always being courteous, responsive, professional, and helpful to the organization’s internal and external customers. 2. Know the contract requirements for your program/service. 3. Provide community living supports and/or services to include, but not limited to: meal preparation, laundry, routine household care and maintenance, activities of daily living such as bathing, dressing, personal hygiene and shopping, money management, non-medical care activities, socialization and relationship building, leisure and community activities, and attendance at medical appointments, etc.4. Provide, as appropriate, personal care supports and/ or services to include, but not limited to: eating/feeding, toileting, transferring, etc.5. As directed, dispense, administer, and monitor medication as indicated, via professional order. Document dispensing and/or administration of medication immediately after the event.6. Participate in Person-Centered Planning meetings, as requested. Make recommendations concerning treatment planning, as appropriate. Assist with the implementation of the persons-served individualized Plans of Service and make recommendations concerning treatment planning to the Clinical Team and/or the Line Manager (in community housing settings). 7. Engage persons-served in problem solving, skill training, and side-by-side activities, as directed (in community housing settings). 8. Lead or participate in the Family/Guardian/Friends Liaison Program activities, building a friendly and open communication network with the natural supports of persons-served (in community housing settings). 9. Engage persons-served and monitor their well being. For residents living in AFC licensed settings, engagement includes monitoring their well being at least every 15 minutes and document activity immediately following the event.10. Assists with emergency situations in an effort to protect and supervise vulnerable persons.11. Document on the progress, or lack of progress, towards goals of persons-served at the close of each shift as necessary.12. Assists with facility and vehicle maintenance, which may include indoor and out of doors tasks.13. Provide and/or assist with transportation related to personal care, community living supports, employment and/or planned activities. 14. Actively monitor and provide feedback on defined line items in budget, as directed by manager. 15. Make sure 100% of the routine/customary daily, monthly, quarterly, and/or annual internal data reports are submitted on time.16. Aid in making sure that 100% of contract/grant compliance reporting requirements are completed and submitted by due date. 17. Adhere to high levels of health and safety practices.18. Make certain that 95% of your training and events requirements are renewed prior to expiration date, as specified by job. 19. Work toward personal and professional goals and objectives. Attend job related in-services and trainings when offered and actively seek to further develop competencies. 20. Adhere to applicable licensing rules, accreditation standards, Codes of Ethics, and New Passages' Policies and Procedures, including Corporate Compliance Plan and Personnel Handbook.21. Sustains “Culture of Gentleness”.22. Attend and actively participate in team staff meetings.23. Perform miscellaneous job-related duties as assigned by Clinical Team and/or Line Manager. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES1. Personable, positive, and enthusiastic attitude, with capability to deal effectively with people (internal and external). 2. Knowledge of characteristics and conditions experienced by consumer population.3. An ability to see the individual value of persons-served and staff peers rather than viewing them only through their organizational role.4. Knowledge of, or ability to learn, the use of technological advances that include, but is not limited to: Microsoft Office Suite (Word, Excel, PowerPoint), Internet, NP Network, fax/copy machines, cell phones, etc. 5. Able to respond in emergency situations.6. Ability to answer a business telephone using knowledge of business procedures and etiquette.7. Ability to schedule/plan work in advance, and set effective and realistic timelines and meet goals. 8. Knowledge of negotiation and mediation techniques.9. Basic knowledge and skill in simple arithmetic, operating household appliances, basic nutrition, cooking, cleaning performing routine household tasks and routine household maintenance. PREFERRED KNOWLEDGE, SKILLS, AND ABILITIES1. Ability to solicit alternate funding through donation solicitation and other fundraising activities. 2. Leadership skillsREQUIRED MINIMAL QUALIFICATIONS1. High School Diploma or Equivalent.2. One year of related experience if working in a Crisis setting.PREFERRED QUALIFICATIONS1. Two years related experience.ESSENTIAL REQUIREMENTS1. Maintain a valid motor vehicle operator license.2. Complete and maintain all required trainings, certifications, and credentials. 3. Maintain professional licensure in an active status without suspension or revocation throughout employment, as applicable. 4. Ability to lift up to 25 pounds.5. Ability to write and/or manipulate a computer keyboard.6. Ability to hear and speak clearly.7. Ability to see both near and far.8. Ability to stoop, kneel, bend, and reach.9.
