Serves as liaison with patient, caregiver, physician, and multi-disciplinary team. Serves as mentor/resource for member of the health care team with regards to orientation and training. Assists staff member in trouble-shooting patient care problems, serving as a resource to other staff. Participates in rounding and related sub-specialty services. Accepts the role and responsibility of charge nurse/clinical lead for practice. Provides ongoing telephonic assistance to patients and caregivers to triage questions, facilitate care, and guide through the health system.Proactively identifies and addresses issues in a timely manner to prevent or decrease acute conditions. Promotes safe, effective, quality care. Provides health teaching appropriate to the patient's developmental level, learning needs, readiness to learn, language preference, and culture. Promotes healthy lifestyle, risk-reducing behaviors, and prevention. Review and develops educational and audiovisual aids for patients/families and members of the health care team. Identifies and implements quality improvements and opportunities for growth. Seeks continuing education and learning appropriate to specialty. Serves as clinic ambassador to publicize the clinic throughout the Beaumont Health System and neighboring communities. Assists in the development of policies, protocols and guidelines related to practice field. Participates in professional development as it relates to the job role. Maintains all necessary documentation of educational offerings prior to submission to leadership. Responsible for working with leadership team in coordinating activities within specialty area. Ability to conduct small scale presentations. Participates in any other clinical activity as indicated for each specific subspecialty service related to management of the chronic disease patients, including but not limited to maintenance and auditing patient medical records.Requirements: BSN required. Active, valid, unrestricted Michigan registered nurse license required prior to expiration date of previous license.
