Provides telephonic symptom management outreach calls to an assigned caseload of Oncology patients while they are receiving chemotherapy treatments. Patient education includes: Regimen specific chemotherapy side effects / Signs and symptoms of potential risks for hospital and ER admissions / Understanding of how to self manage symptoms at home / Basic introductions and counseling sessions focused on Advance Care Planning. The RN will provide verbal and written communication with clinic RNs and Payer Case Managers. All patient and clinic interactions are recorded in the clinic Electronic Health Record. Supports and adheres to the McKesson Compliance program, to include the Code of Ethics and Business Standards as well as all HIPAA protocols and client contractual obligations.
Responsible for providing telephonic outreach assessment calls to Oncology patients including reviewing of Electronic Health Record in preparation for calls
Responsible for providing telephonic Advance Care Planning counseling sessions with appropriate Oncology patients.
Ensures timely and accurate documentation in Innovent IT systems, and clinic Electronic Health Record
Communicates professionally with clinics and payer case managers
Documents in multiple Excel Spreadsheets to assist in gathering data
Responsible for accurately opening and closing cases which triggers important elements for the billing process
Minimum Requirements: (job code specific)
5 years nursing experience
2 – 3 years experience in an oncology practice in the capacity of direct patient care
Telephonic triage experience
Chemotherapy infusion experience within the past year
Patient education experience
Critical Skills: (must have)
Excellent communication and listening skills
In depth understanding of overall cancer process and side effects of chemotherapy
Excellent documentation skills / Electronic Health Record
Flexible and quickly adapts to change
Organization and Time Management
Proficient in Computer skills: Word / Outlook /
Additional Knowledge/Skills: (preferred)
Basic Microsoft Excel
Education: (job code specific)
Graduate from an accredited program for professional nursing education
Current active RN License in state of Texas
Ability to obtain licensure in additional states if needed
