Job Description

Community Link Program specializes in private and public social and community-based programs offered at the national, state and local level. This role is responsible for researching programs and validating benefit descriptions, eligibility requirements, contact information and applicable forms, and any other information related to community programs available to our customer health plan members at the state and federal levels.


  • Conduct research regarding available private and public social and community-based programs via internet searches, telephone contacts, client suggestions and outreach unit recommendations.

  • Assess quality of researched programs and make recommendations to add to Community Link’s program library.

  • Analyze Community Link’s program library at scheduled intervals to identify gaps offerings based on the client member demographics.

  • Obtain copies each community program application and coordinate its inclusion of programs into Altegra Health’s proprietary Medicaid Management System (MMS). Obtain updated versions of the community program application forms as necessary and ensure they are promoted accurately and timely into MMS so that they are made available to staff.

  • Maintain all community program information to ensure it is up-to-date and accurate. Perform program QA and review for other content specialists. Perform post-production validation of programs including forms testing and program interview testing.

  • Create Community Link referral forms and reference materials. Update the reference materials as needed with changes to the income and/or asset requirements for each Community Link program

Job Requirements

  • HS Diploma/GED required. College degree/some college coursework preferred

  • Strong hands-on experience performing internet searches

  • Ability to write and create concise, clear descriptions of Community Link programs

  • Some understanding or experience in working in a social advocacy role desired

  • Excellent written and verbal communication skills.

  • Advanced researching/research analysis skills

  • Computer Software Skills – Proficient in Microsoft Office Suite and Microsoft Outlook
